You have the option of completing the form in its entirety as a hard copy (print out and fill in) OR fill out using google forms- electronically (then the link).
Please be reminded that you still need to attach all documentation in the google forms- in the upload sections. So, take pictures to upload to this site, if this is the desired way you wish to submit your intake application. Example: Take a photo of the status card and upload it in the appropriate sections.
